Vitamin D and Irregular Periods What I Want My Patients to Know

In my experience working with women with PCOS, vitamin D deficiency is something I see very frequently. When a patient comes to me with irregular periods, checking vitamin D status can be an important part of understanding the bigger picture.

Why Vitamin D May Matter in PCOS

Vitamin D receptors are present in the ovaries and uterus, and vitamin D plays a role in several processes involved in reproductive and metabolic health.

Low vitamin D levels have also been associated with hormonal imbalance and insulin resistance, both of which can be relevant in women with PCOS and may contribute to menstrual irregularity.

That said, I wouldn’t recommend assuming that vitamin D deficiency is the cause of irregular periods. PCOS and menstrual changes can have multiple contributing factors, so proper evaluation is important.

What I Recommend to My Patients

1. Get Some Sun Exposure

When appropriate for your skin type and local conditions, spending some time outdoors can support natural vitamin D production. The amount your body produces varies considerably depending on factors such as skin tone, season, location, time of day, and sunscreen use—so there isn’t one sun-exposure duration that works for everyone.

2. Include Vitamin D–Rich Foods

I encourage my patients to include foods such as egg yolks, fatty fish, and fortified foods where appropriate. These can be a useful part of an overall balanced diet.

3. Consider Supplementation When Needed

If blood tests show a deficiency, supplementation may be recommended. The dose should depend on your actual vitamin D level and individual circumstances, rather than taking a high dose automatically.

Look at the Bigger Picture

Correcting a vitamin D deficiency can be important, but it shouldn’t be viewed as a standalone treatment for PCOS or irregular periods.

In my practice, I prefer to look at the whole picture—menstrual patterns, hormones, nutrition, insulin sensitivity, lifestyle, and overall health—and then build a plan around the individual patient.

If your periods are irregular, don’t just treat the symptom. Find out what’s driving the change.
